Directions: Please list the names of family members who passed away since last Yom Kippur. If any of the names listed are on a plaque in our Chapel’s Memorial Tablets, please write “Tablet” next to the name.

Names in last year's booklet (2024) will be automatically put in this year's booklet (2025). If the names on your list are the same as last year, you do not need to add them.

NOTE: Submissions must be received by SEPTEMBER 10th to be included in the book.
